"Two Plus Two" Program
By enrolling in the "Two Plus Two" IVF program you will pay a designated fee for your first and second IVF cycles that is $1,500 higher per cycle than our traditional fee-for-service rate. If you fail to become pregnant after completing your second IVF cycle with the "Two Plus Two" Program, you then become eligible for up to two more cycles at no additional cost to you.
This program allows our patients to self-insure against the financial risk of having to undergo multiple cycles and greatly reduce costs if three or four cycles become necessary to achieve a pregnancy. If you participate in the "Two Plus Two" Program and do not become pregnant after the second cycle, the cost to you would be $6,000 per cycle if you require a third cycle, and $4,500 per cycle if you require a fourth cycle. This option was designed to allow patients on a limited budget more opportunities to achieve a successful pregnancy should they fail to do so in their first two IVF attempts at AFS.
Who Qualifies
Qualification Criteria: "Two Plus Two" Program
The "Two Plus Two" IVF Program is offered to all women and couples who are candidates for our regular fee-for-service IVF treatment. Unlike some other fertility programs, AFS does not automatically disqualify women from attempting IVF if their chances for success are below average but still possible, but we will fully apprise you of your realistic chances for success given your medical profile before you participate in our program.
What's Included and Not Included
"Two Plus Two" What's included
- 4 IVF cycles (fresh)
- Ultrasound and cycle monitoring
- Egg aspirations (retrieval)
- Intracytoplasmic sperm injection (ICSI)
- Assisted hatching (AH)
- Pregnancy monitoring only until fetal heartbeat, number of fetuses, and position are established by ultrasound
"Two Plus Two" What's not included
- Ongoing pregnancy monitoring *
- Diagnostic procedures such as hysteroscopy, laparoscopy, etc. *
- Prescreening tests *
- Hospitalization in the event of complications *
- Semen analysis *
- Urological procedures such as testicular biopsy, donor sperm, etc.
- Egg donation
- Anesthesia **
- Medications *
- Blood work *
- Cryopreservation
- Frozen embryo transfers
- Sex Selection
- PGD
* Many of these may be covered by an insurance plan, even if you do not have coverage for IVF
** At present, this cost is approximately $500 per procedure requiring sedation
Fees
Fees: "Two Plus Two" Program
- $10,000 per IVF cycle for the first two IVF cycles (Please note that this is not a fee refund program)
- If patient achieves pregnancy after the first cycle no further "Two Plus Two" cycle payments to AFS are necessary
- If patient does not achieve pregnancy after 2 IVF cycles, she is entitled to 2 cycles at no additional fee
* Please note procedures/tests, as listed above, are not included in the program
